Information that we need before the interview …

We ask our patients to complete the intake forms as thoroughly as possible. The more information you are able to provide, the more help we can offer. There will be questions about the disability, general health, diet and nutrition, lifestyle, habits, personality, and life stresses that the patient may be experiencing.

Here is a list of the required forms that must be completed:

1. FORM “A” - DOCTOR’S RELEASE FORM

   Form "A" should be completed by your health care professional. It indicates that the individual is medically fit to participate in a therapy program. Please print it out, have your doctor sign it and return to our offices by mail or fax.
   
   
2. FORM “B” - ACKNOWLEDGEMENT FORM

   Please print it out, sign it and return to our offices by mail or fax. It indicates acceptance of the terms of our engagement.
   
   
3. FORM “C” - PATIENT HISTORY FORM

   Form "C" provides us with patient history and background information. This form can be completed online. As an alternative, it can be printed out, completed and faxed or mailed to our offices.

Please bring any medical reports regarding health, including blood tests, genetic tests, x-rays, MRI’s, CAT scan, ultra-sound, thyroid tests, psychological or educational evaluations, etc…. Further information may be requested, if required.
